Hillcrest Community Support is a flexible service providing support staff to individuals looking to improve thier social networks and establish meaningful day time activities or routines.

Assessment
Personal contact is made with the service user and their families to find out what support is required, what they want to achieve through support and how they wish to be supported. We ensure that we maintain communication with the service and their families so we can be responsive to changes in need.
Support Plan
The service user, their family and multi-disciplinary support network are fully involved in constructing the support plan. The support plan can be completed by the service user if they prefer or by going through each section with a member of staff.
Your Support Team
The service user is involved in selecting the members of staff to support them through a process of matching their preference hobbies and interests. Matching a dedicated staff team to each service users helps promote consistency, enables the service users to feel in control of who supports them and ensures that staff availability is matched to the times when support is required.
Funding
All support is agreed on a spot purchase basis and can be purchased through the Local Authority, Individual Budget, Direct Payment or Independent Living Fund.
